Cirrhosis is a chronic liver disease where scar tissue replaces healthy liver tissue.
Alcohol abuse, Hepatitis B / C and now non-alcoholic fatty liver are the commonest
causes of cirrhosis. Its an irreversible disease with no cure. By the time diagnosis
is made more than 70% of the liver is not functioning. It can remain asymptomatic
or may present as swelling of feet, distension of abdomen due to accumulation of water,
profound weakness, poor appetite, jaundice or vomiting of blood. If not diagnosed in
time before complications, mortality can be as high as more than 60% in five years.
The common causes include chronic Hepatitis B or C, long-term alcohol use, non-alcoholic fatty liver disease (NAFLD/NASH), and autoimmune or metabolic liver diseases.
Early stages may have no symptoms. Some patients may feel tired, weak, or notice mild
swelling in the legs.
Advanced cirrhosis can cause jaundice, swelling of the abdomen (ascites), bleeding
from the stomach or food pipe (varices), confusion (hepatic encephalopathy), and risk of liver cancer.
Cirrhosis is often suspected when patients have risk factors like hepatitis, fatty liver,
or alcohol use, or when abnormal liver tests are found. Diagnosis involves a combination of:
Cirrhosis treatment consists of close follow up of the patient, diatary restrictions on salt intake, high protein diet, nutritional support. Early detection and prompt treatment can offer better quality and prolongation of life. Liver transplant is the treatment for decompensated disease. It also carries risk of developing into cancer. Regular follow up and sonography surveillance is required.
